Refactor code to correct JSLint "undefined function" warnings. Note: diffs will be large due to code reordering.

Native XDR transactions now fallback to io.swf(Flash transport) if the browser cannot initiate such a require and throws an exception. Serialised form data can now be sent in XDR transactions -- Fixes 2528167.

Fixed bug in io-form: form nodes without a defined id attribute will now receive a custom id, before form serialisation.

Fixed incorrect retrieval of id from node in io-form.

io-form now accepts a form node as well as an HTML form id.

Update io-base.js to synchronize with io-xdr.js when calling isInProgress for cross-domain transactions. Removed redundant semi-colons from all io sub-modules.
