auth: Code cleanup: Merged extra_cache_fields into extra_fields. They are separated using a hidden-flag in the extra field. This required a new implementation for auth-streams.

Avoid using (void)s by adding ATTR_NOWARN_UNUSED_RESULT attributes and other ways.

Make sure we check all the functions' return values. Minor API changes to simplify this. Checked using a patched clang that adds attribute(warn_unused_result) to all functions. This commit fixes several error handling mistakes.
